Every child must have a new enrollment form completed each year. 


Completing and submitting this registration form, along with the non-refundable registration fee (to the director), will confirm placement.  In order for your child to attend, we will also need the following forms completed and submitted on or before, August 1st of the new school year:

Permission to Assist with Toileting


In addition to these forms, you will need to complete a google form online.  The link to the appropriate form for your child's class will be emailed to you before classes begin.

  • Child health form

    This document needs to be completed and signed by a physician along with a list of child's immunizations and submitted ON OR BEFORE August 1st.  

  • Medical consent and

    permission to assist with toileting

    This form must be completed and signed by a parent and submitted ON OR BEFORE August 1st.
