Registration Procedure

Please click on the red rectangle in the picture below to open our registration form which you can fill out electronically, print and return to us. 

Every child must have a new enrollment form completed each year. Completing and submitting this registration form, 

along with the non-refundable registration fee (to the director), will confirm placement. In order for your child to

attend, we will also need the following forms completed and submitted on or before, August 1, 2022: 

Physician Statement and Immunization Report  

Emergency Medical Form

Student Information Sheet

Permission to Photograph

Installment Agreement


Permission to Assist with Toileting
